What is my Unique Facility Identifier and how do I get one? Can I use my IRS number, FDA Registration number, or FDA Establishment Indicator?

 

The FSVP importer must be identified on the U.S. entry. One of the identification elements which you may have heard about is the Unique Facility Identifier (UFI) or DUNS (Data Universal Numbering System) number. The DUNS number is currently the only UFI approved by the FDA. You cannot use your IRS number, FDA Registration number, or FDA Establishment Indicator. The DUNS number is the only acceptable Unique Facility Identifier.

The DUNS is a numerical identifier assigned to a specific business location. The DUNS number is assigned by Dun & Bradstreet, and can be applied for by one of the following options:

 

For US Importers with Multiple Locations

D&B D-U-N-S® Number

“A non-indicative, nine-digit number assigned to each business location identified in the D&B database as having a unique, separate, and distinct operation.”

For example, if ABC Pets and Washington Pet Supplies are both at 123 Main St, Seattle, WA, there would be two DUNS numbers assigned, one for each business unit.

If Stevenson Pianos had 14 locations, each location would have a separate DUNS number.

To properly report the correct DUNS number and address of the US Importer in multiple location scenarios, you will need to provide the DUNS number, name, and email address for the FSVP Contact at the location chosen by the FSVP Importer.
